   3 year old class (T, TH  8:30-11:00 a.m.)          $100 mo.           3 by August 1, 2013
   4 year old class (M, W, F  8:30-11:00 a.m.)     $138 mo.          4 by August 1, 2013
   4/5 year old class (M-TH  12:30-3:00 p.m.)    $180 mo.          4 by January 31, 2013

The registration includes an Application for Enrollment, a Child’s Health Record, and a Medical Emergency Authorization Form.
